  1. When booking dates for psychological and psychiatric consultations and selected surgical appointments<\/u>, a deposit is required.<\/li>\n
  2. The amount of the deposit is PLN 100. On the day preceding the consultation, the CM PROKOCIM reception will send a link to quick payments (i.e. blik, payU, etc.) to pay the deposit.<\/li>\n
  3. In order for the reservation to be considered binding, a payment must be made. You can also pay a deposit to the clinic’s bank account, and in the title give your name and surname and send a confirmation to the clinic’s e-mail address.<\/li>\n<\/ol>\n

    1. The difference between the deposit and the entire amount for consultations is paid by the patient only after the consultation on the spot.<\/li>\n
    2. If the patient cannot report at the agreed time during the visit, he or she is obliged to
      \n report this fact<\/u>
      \n<\/strong> no later than on the day of the visit by 08:30 a.m<\/strong> . At a later date, this deposit
      \n is forfeited<\/u>
      \n<\/strong>.<\/li>\n
    3. If the booked appointments are not cancelled with a specified notice (i.e.<\/strong> by 08:30 on the day on which the appointment is to take place<\/strong>), it is considered used and the deposit is<\/strong><\/u> not<\/u>\n

      <\/strong> returned to the patient.<\/li>\n

    4. If the patient resigns from the appointment sufficiently in advance<\/li>\n<\/ol>\n

      (i.e<\/strong>. before 8.30 a.m. on the day of the visit<\/strong>), the deposit paid can be used within 2 months from the date of the missed visit<\/u>.<\/p>\n
