Booking Terms and Conditions

1. When booking dates for psychological and psychiatric consultations and selected surgical appointments, a deposit is required.
2. The amount of the deposit is PLN 100. On the day preceding the consultation, the CM PROKOCIM reception will send a link to quick payments (i.e. blik, payU, etc.) to pay the deposit.
3. In order for the reservation to be considered binding, a payment must be made. You can also pay a deposit to the clinic’s bank account, and in the title give your name and surname and send a confirmation to the clinic’s e-mail address.

4. The difference between the deposit and the entire amount for consultations is paid by the patient only after the consultation on the spot.
5. If the patient cannot report at the agreed time during the visit, he or she is obliged to
   report this fact
   no later than on the day of the visit by 08:30 a.m . At a later date, this deposit
   is forfeited
   .
6. If the booked appointments are not cancelled with a specified notice (i.e. by 08:30 on the day on which the appointment is to take place), it is considered used and the deposit is not

   returned to the patient.

7. If the patient resigns from the appointment sufficiently in advance

(i.e. before 8.30 a.m. on the day of the visit), the deposit paid can be used within 2 months from the date of the missed visit.

8. Prokocim Medical Center recognizes the following form of cancellation of the booked date, i.e.: in person, by e-mail, by phone.
